Late 90's Movie where a teen killer who slits throats of women sees a psychiatrist and kills her at the end.

The movie I'm trying to find follows a blonde psychiatrist who takes on a boy who looks like Johnny Galecki as a client. The teen is a serial killer who slits the throats of women and this terrifies the woman and she has a dream about him killing her. He has sex with a woman but doesnt kill her. Instead je stares out a window naked. At the end he slits the psychiatrists throat and dark blood flows out of her neck before credits roll.
