Sundance Movie I Watched In Mid 90'S; Girl Takes Pictures Of Her Friend In The Nude And Then Her Friend Wants To Take Pictures Of Her

I only saw a few pieces of this movie. I watched it on Sundance in the mid 90's, but it looked like it was made in the 80's. It was in English, but the actors had accents so it was some European film; not sure which country.  I believe the beginning was in a hospital with a doctor doing some brain surgery. There's a shot the doctor sawing open a skull of the patient. I remember it looked like was using a piece of rope held at both ends and pulling it with one hand and then the other.

But there's only one scene I really remember.

The main character is getting her picture taken in the nude by her friend.  After a while, she mentions how its her turn to take pictures of her friend.  The friend says no and that she's not comfortable being naked. The main character chases her around a little and basically forces her clothes off her. The scene flashes forward and the friend is fine being naked, but then this guy shows up at the apartment. I remember the friend running around frantically trying to grab cushions or blankets to cover herself.

I am pretty sure you are speaking of The Unbearable Lightness of Being (1988).
